How to fill in the sheet
Each player has a column of thirteen boxes to fill in thirteen turns. On each turn, up to three rolls, then you must write a result in an empty box — a zero if nothing fits. In the upper section (aces to sixes), write the sum of the dice showing the chosen value: three 5s in the fives box make 15. Add up the upper section: if it reaches 63, add the 35-point bonus on the line provided.
In the lower section: three of a kind and four of a kind score the sum of all five dice; full house 25, small straight 30, large straight 40, Yahtzee 50, chance the sum of the dice. The grand total is the sum of both sections; the highest wins. If you play with the 100-point bonus for additional Yahtzees, note it on the Yahtzee line.

| Twos | 2-2-2-5-6 | 6 |
| Threes | 3-3-3-3-1 | 12 |
| Fours | 4-4-4-2-5 | 12 |
| Fives | 5-5-5-5-3 | 20 |
| Sixes | 6-6-6-2-2 | 18 |
| Upper section | 70 ≥ 63 | 70 + 35 = 105 |
| Three of a kind | 4-4-4-6-5 | 23 |
| Four of a kind | — | 0 |
| Full house | 2-2-5-5-5 | 25 |
| Small straight | 1-2-3-4-4 | 30 |
| Large straight | 2-3-4-5-6 | 40 |
| Yahtzee | — | 0 |
| Chance | 6-6-5-4-3 | 24 |

The four 5s (20 points) gave 5 points of credit over the 63 threshold, which made up for the poor aces box.
Mistakes to avoid on paper
- Forgetting the 35 bonus in the final sum: it matches no roll, so nobody sees it.
- Granting the bonus at 62: the threshold is 63.
- Replaying a filled box “because it was a zero”: a box is filled once.
- Scoring three of a kind as 3 × the value instead of the sum of all five dice.
